Here’s your opportunity to get your answers right away, make sure you’re on track, and bulletproof your recordkeeping. Program Goals/Objectives: At the end of this program, the attendee will be able to: 

 

  • Understand the mechanics of the beginning of an episode of care from initial history to exam, diagnosis, and treatment planning
  • Master the requirements for daily treatment notes and re-evaluation according to Rule 22 guidelines
  • Establish medical necessity for care and understand whether that care meets the standards for third-party reimbursement, or is maintenance, wellness, or other forms of non-third-party reimbursed care
  • Understand the requirements for documenting primary subluxations vs. secondary compensations, especially when technique-specific protocols dictate multiple regions of treatment
  • Interpret compliance advice and regulations directly from the Federal Office of Inspector General; when we’re operating in compliance, it’s much easier to sleep at night

 

Program Outline:

 

Hour One: Documentation Overview, Definitions, and General Requirements

  • Signatures, legibility, and other basics of documentation
  • The layers of documentation and how to ensure nothing is missed
  • The difference between clinically appropriate care vs. medically necessary care when documenting the daily execution of your treatment plan for a cohesive account of the patient’s episodic journey

 

Hour Two: Case-Management-Driven Documentation

  • Report the mechanics of moving a patient from the beginning of an episode of care to initial history, exam, diagnosis, and treatment planning through daily visits that tell the story of the patient’s progress
  • Illustrate how proper case management meets federal documentation requirements for Medicare and other third-party payers, while documenting key components and milestones of “doctor thinking” activities
  • Understand the difference between incidents, bursts, and episodic care, including knowing when one ends and another begins
  • Understanding the transition between evaluation, management, and treatment

 

Hour Three: Risk Management and Recordkeeping Assessment in Federal Programs

 

  • Additional Medicare documentation and management requirements
  • Overview of the PART process for documenting subluxation
  • Clarity about Medicare’s definitions of medical necessity as well as acute, chronic, and maintenance care

 

Hour Four: Tie It All Together for Excellent Documentation

 

  • How Colorado’s rules and Medicare’s regulatory requirements apply to recordkeeping
  • Performing internal audits of your documentation to confirm requirements are met
  • OIG compliance and risk management as it applies to documentation
