 Documentation:

The break down of Rule 22 
 
Part I: An effective patient history. The proprer documentation of the chief complaint, how it began, what provokes it , what relieves it, how to grade the quality of the patients chief complaint and when and how often it occurs.  The history taking section will include documenting patients past and present illnesses, injuries and surgeries, along with review of system procedures.  This course will teach the recording of non-prescription and prescription supplements etc and correlating the necessity of documenting these elements. 
 
Part II. Demonstration of how to quickly and thoroughly perform and document chiropractic, orthopedic, neurologic, physical and clinical examinations findings and guidelines.
 
Part III. The correlation of a patient history with exam findings to create justifiable case management and clear clinical decisions.
 
Part IV. Documentation of pertinent patient interactions including, in person and phone dialogue, doctor visits, along with emergency and hospital visits. 
 
Forms on some presented material will be presented.
 

Instructor:

 

Galen G. Graham D.C. began his chiropractic practice in Colorado Springs in May of 1973 and has been a licensed chiropractor since 1973.  He was honored with an appointment to the Colorado Chiropractic Board of Examiners by Governor Lamb in 1978 and served as President of the Board from 1982 – 1983.  Dr. Graham completed 300 hours proficiency in x- ray and an additional 300 hours in orthopedics and is Board qualified in both subjects.  He has held classes in conjunction with the Colorado Chiropractic Association and has personally presided over 10 or more certification classes approved by the Colorado Board of Chiropractic Examiners in documentation. 

His other contributions to the betterment of the profession are too numerous to mention.
